Appointments
After you complete the consultation request, I will reach out to you by phone or email. The initial evaluation is a 80 minute appointment where I will complete a comprehensive evaluation of your medical and psychiatric history and your reasons for seeking treatment. The consultation is an opportunity for us to decide whether we are a good fit for ongoing care. At times, it may take several sessions to determine if the treatment relationship is a good fit. If we decide to proceed, we will collaborate together to form a treatment plan which may include psychotherapy, medications, couples therapy, or a combination of treatment approaches. The type and frequency of treatment may change over time.
Payment of Fees
I am out-of-network for medical insurance companies. I can provide you with receipts with a diagnosis and treatment code which you can use to obtain reimbursement from your insurance carrier, if you have out of network benefits. You can also use a health care savings account to offset the cost of treatment.
Medicare patients should know that I have "opted-out" of Medicare. Therefore, I am no longer a Medicare provider, and neither Medicare nor Medigap insurance programs will reimburse for the cost of visits.
Missed Appointments/ Cancellations
If you are unable to keep an appointment, please give at least 24 business hours advanced notice . Appointments missed or cancelled with less notice will be charged for the full appointment fee.
Confidentiality
In general, your permission is necessary for me to share your medical records and information for any reason, unless I suspect you are in imminent danger of harm to yourself or others, or a child or elderly person is being abused or neglected.
