Clinical Info
If you take medications for mental health, please list each medication (and dose) that you currently take. (If not applicable, write N/A). *
In my practice, I offer talk therapy and medication if needed. I have limited spots for medication only visits. The frequency of meetings depends on your particular needs, but ranges from every week to every 3 months. My practice does not have capacity to offer acute crisis management services. The fee for a follow-up session is $325. I do not accept insurance but can give a receipt for you to submit to your insurance, if your plan offers out-of-network reimbursement. As per licensing laws, I can only care for individuals who reside in Massachusetts at the time of the appointment. Are these aspects of my practice logistically feasible for you and in alignment with your goals? (Yes /No answer). *
If you are looking for medication management alone, what are the names of the people on your treatment team? Please include group therapy, coaching, regular therapy, etc. (I like to know who these people are to make sure my philosophy of care aligns with their approach). *
Please share briefly your mental health goals. What do you hope to accomplish? Is there anything you find esepcially unhelpful when it comes to mental health treatment approaches? *
Scheduling Info Currently most appointments are morning or early afternoon times, but sometimes exceptions can be made. Appointments are often 45-50 minutes. If you can't make morning or early afternoon times, what days/times DO work for you to meet virtually? *
Currently the practice offers mostly telehealth (virtual) visits by video. There are a limited number of in-person visits (west Cambridge location) available intermittently on Wednesdays. Are these logistics workable for your needs? (Yes/no) *
Thank you for your courage in reaching out, and for your time in completing these questions! Here are the next steps of the process: 1. You will be asked to create a login and password to Luminello. This may seem cumbersome, but is required in order to follow mandated HIPAA privacy laws. 2. Then, Dr. Warren will review your request. If she has openings and thinks her practice may be able to address your needs, you will receive a message from her with more details. 3. If Dr. Warren determines that her practice does not have expertise or capacity to address your needs, you will receive a message letting you know. In this case, please do not give up in your determination to find treatment. It can be overwhelming and confusing to navigate treatment options. Here are some suggestions: -Ask PCP for recommendations -Call your health insurance for a list of covered providers -Talkiatry.com -Also, group treatment options may be less costly and more quickly accessible: -Depression/bipolar support groups: https://www.dbsalliance.org/ -Mindfulness groups: chacmc.org & centerformsc.org -Facilitated Support Group https://pace.group -Suicide prevention: https://www.spnawareness.org/ with best wishes, Laura Warren, MD (space below can be left blank or you can add any additional questions/concerns here) *
