Greetings from Dr. Riegel! The process to start the intake: - Please complete our pre-screening - You will receive an email after we have reviewed the pre-screening with next steps. If it seems like we may be a good fit for each other this email will allow you to schedule an appointment. For patients who are transferring from Munson Health Care and have previously been patients of Dr. Riegel, please schedule a "Medication Review." For patients who haven't been seen by Dr. Riegel before or in the last several months, please schedule a "Consultation." Before your appointment, please complete the following items at least 24 hours prior to your appointment time or you may be subject to your appointment being canceled and you may be charged up to the full appointment cost. - Intake Questionnaire - Intake Paperwork (polices, procedures) Please note that all scheduling and rescheduling is done online through Luminello via your new portal (https://app2.luminello.com/auth/login). If you need to reschedule please do so >48 hours prior to your appointment time.
